Overview of Chondroitin Sulfate and Cartilage Protection Chondroitin sulfate does not regrow lost cartilage. No oral supplement or current non-surgical therapy has demonstrated cartilage regeneration in humans. However, clinical evidence supports a protective role: pharmaceutical-grade CS at 800–1,200 mg/day has been associated with reduced cartilage loss and slowed joint space narrowing in mild-to-moderate knee osteoarthritis over 2-year studies. CS acts through anti-catabolic, pro-anabolic, and anti-apoptotic mechanisms, though these are primarily established in vitro. Imaging studies show slowing of degradation, not increased thickness. Response varies; onset takes 8–12 weeks. Product quality is a critical variable; pharmaceutical-grade material with verified purity and molecular weight is distinct from many supplement-grade products. CS is adjunctive to exercise and weight management, not a replacement.

Overview of Chondroitin Sulfate Molecular Weight Chondroitin sulfate is a polydisperse polymer with chain length varying by source and processing. Native CS in cartilage is 50–100 kDa; after extraction, it typically falls to 10–50 kDa. Shark-derived CS is higher (50–70 kDa), while bovine and porcine sources yield 14–26 kDa. Low molecular weight CS (<10 kDa) is used in cosmetics and transdermal products. Molecular weight affects absorption, with lower fractions showing greater intestinal permeability. Pharmacopoeia standards vary: China specifies Mw 35–50 kDa for injectable CS, while USP focuses on purity without explicit MW limits. Polydispersity index (PDI) is a critical quality parameter; a narrow distribution indicates batch consistency. Sourcing should define MW range, PDI target, and verification method.
